Filing a Report

Consent to the Collection and Use of Personal Information

Please carefully read the information below regarding the gathering and use of personal information before deciding whether or not to report through Ethics Hotline.

1. Why personal information is gathered and used: Processing of the consultation and response to provide information about the outcome
2. Items of personal information collected
- Required items :
  Name, any and all personally identifiable data contained in the report (contents and attachments you submit, such as the report's type, the target company, the incident's history and specifics), as well as the relationship between the informant and the company
- Optional items :
  Mobile phone number and email address
3. After processing the consultation and sending the result back, personal information is retained and used for a maximum of three months.
4. The right to refuse consent and the disadvantages of refusal
You may refuse to provide consent regarding the collection and use of personal information. However, if you refuse to agree to the collection and use of personal information, there may be service(some) restrictions such as replies regarding reception/processing results.
